What You'll Do
- Deliver skilled nursing care in patients' homes across Mesa County
- Complete assessments, develop care plans, and coordinate services
- Monitor and evaluate patient progress, updating plans as needed
- Collaborate with nurses, therapists, caregivers, and families
- Provide education to patients and families to promote independence and recovery
- Document visits and communication in EMR accurately and timely
- Competitive hourly pay based on experience
- Monday–Friday, 8 AM – 5 PM schedule, Rotating Weekend Call
- Mileage reimbursement for all travel

- Active RN license in the State of Colorado
- Experience in home health preferred, but we will train the right candidate
- Excellent clinical judgment and strong documentation skills
