A social services organization in New York City is actively seeking a dedicated and personable professional to join one of their facilities as a Wellness Coach.

About the Opportunity :

  • Schedule : Saturday to Wednesday
  • Hours : 2pm to 10pm
  • Setting : 45-bed co-ed community facility
  • Population : High-risk

Responsibilities :

  • Provide case management and develop individual support plans
  • Assist with daily living skills and crisis intervention
  • Accompany residents to medical / psychiatric appointments
  • Facilitate community integration activities

    • Qualifications :

  • 2 years of experience working with the Behavioral Health population
  • Associate's Degree
  • Solid assessment and documentation skills

    • Desired Skills :

  • Bachelor's Degree
  • Bilingual (English and Spanish)
  • Experience working with the Mental Illness, Substance Use, and Homelessness populations
