Role Summary
We are hiring a Community Navigator (Support Coordinator) to support individuals in New Castle County. This role involves assessing client needs, developing service plans, and connecting individuals to the resources and supports they need to live fulfilling lives.
Key Responsibilities
- Conduct needs assessments in areas such as health, social, employment, and personal well-being
- Develop and implement individualized service plans
- Monitor service delivery and evaluate effectiveness of care plans
- Make recommendations for services including clinical treatments, benefits, or counseling
- Interpret and apply relevant laws, policies, and procedures
- Enter and manage case data using an automated system
- Write clear and concise narrative reports
- Travel within the community to meet with individuals as required
Required Qualifications
- Associate's Degree - Required
- Minimum 1 year of experience in health or human services support, including interviewing and needs assessments
- At least 6 months of experience in case management including planning and implementation
- 6 months of experience making recommendations for client service plans
- 6 months of experience interpreting laws and policies
- 6 months of experience with automated information systems
- 6 months of experience in narrative report writing
- Valid state driver's license, automobile insurance, and access to reliable transportation
Preferred Qualifications
- Bachelor's degree in Human Services or related field
- Experience working with individuals with developmental or intellectual disabilities
