Minimum Qualifications
- Active CDR credential (Required)
- Florida Dietitian/Nutritionist license or eligibility
- 1–2 years of experience in Long‑Term Care or Rehab settings (minimum)
- 3–5 years of experience preferred
- Strong communication and collaboration skills within clinical teams
- Knowledge of regulatory guidelines for nutrition and dietary services
- Experience with:
- CMS, ACHA, LTC, SNF, Rehab
- MDS and Care Plans
- Point‑Click‑Care
- Resident‑centered care models
- CMS/ACHA survey processes
- Ability to work effectively with facility care teams
- Conduct comprehensive nutritional assessments for residents
- Develop, implement, and monitor individualized nutrition care plans in accordance with regulatory standards
- Collaborate with nursing, therapy, and dietary departments to support optimal resident outcomes
- Provide education to staff, residents, and families on nutrition‑related topics
- Participate in care plan meetings and maintain accurate, timely documentation
- Ensure compliance with state, federal, and organizational dietary regulations
