Social Worker

Responsible for the provision, direction, supervision, and evaluation of medical social services to patient/client in the home in accordance with Agency policies and procedures, applicable healthcare standards, governmental laws and regulations. 

QUALIFICATIONS:

Educational and/or Professional Qualifications:

1. Must have a master’s degree from a school of social work, accredited by the Council on Social Work Education, and be Board Certified in Social Work.

Previous Experience:

1.   Minimum two years of experience preferably in home health environment or related field.

Licensure, Certification, Registration, etc.:

1. Current Louisiana licensure/certification as a Social Worker required.
2. Certification in Basic Life Support for Healthcare providers by American Heart Association required first of the month following 90 days of hire.
3. Valid driver’s license required.
