What are the responsibilities and job description for the Family Service Coordinator position at Tresco, Inc.?
Summary: Develop and lead a team of Home Visiting professionals in providing quality enrollment and eligibility services in accordance with the policies and practices of Tresco, laws, regulations and administrative rulings of governmental organizations and other regulatory and advisory authorities and organizations.
Responsibilities
- This position requires that the employee comprehend the English language (read, write, or speak).
- Coordinate accurate intake, evaluations and assessments and determining eligibility within required timelines.
- Facilitate and actively participate in the development of the initial and annual Individual Family Service Plan (IFSP) and the 6-month review of the IFSP within required timelines.
- Facilitate and actively participate in the initial and exit ratings for the Early Childhood Outcomes.
- Assist families in identifying and accessing all available services and resources, not just those related to the child’s condition (e.g. housing, mental health services etc.).
- Coordinate and monitor the delivery of services (including subcontractors and providers from other agencies).
- Maintain accurate children’s working files as per accreditation standards and Medicaid.
- Provide appropriate client data to be entered in the data base in a timely manner.
- Inform families of advocacy services and empower the family to enhance their own Family Service Coordination skills.
- Coordinate services for families with medical and health providers.
- Facilitate the development of a transition planning process for each child and family within required timelines.
- Coordinate discharge of children from program within required timelines.
- Gather and research resource information for the family and make referrals where appropriate.
- Maintain on-going register of resources and services in the community.
- Attend and successfully complete required Family Service Coordination trainings (online and classroom) within one-year of hire.
- Attend and successfully complete on-line Family Service Coordination training (on-line training without credit) every three years to remain current on revised requirements.
- Actively participate in reflective supervision at least once every month.
- Maintain clear records of time spent with and on behalf of families for audit purposes by accurately completing daily activity logs.
- Accurately provide separate and distinct activities documentation when providing a dual role, (i.e. another service to the child and family such as special instruction, speech therapy etc.).
- Provide a minimum of one (1) accumulated hour of Family Service Coordination in order to be reimbursed for that month. Documentation must include all time spent with the family and work done on behalf of the family, including work beyond the one hour minimum. Travel time may not be included.
- Assist in collecting and monitoring data for billing and budget purposes.
- Assist in organization of program activities and child find opportunities.
- Attend and actively participate in all scheduled meetings.
- Accept and implement special projects as assigned by supervisor.
ADDITIONAL DUTIES. Other job-related duties as assigned.
Qualifications
Education. Bachelor’s degree in social work, counseling, psychology, nursing, special education, early childhood education or closely related field.
Licenses/Certifications. Valid driver’s license.
Other (e.g., post-offer drug screen, background check, driver’s license record, etc.). Employee’s must undergo a driver’s license record check; employees must pass a background check. Note: For Federal position list NCICS Clearance

Rate of Pay: 15.00 - 19.00
Location: Anothony, NM
Pay Range: SP-1
Hours: Mon-Fri 6:00am-3:00pm
