Job Description:
We are seeking a dedicated and compassionate Cardiac Rehab RN/Exercise Physiologist to join our team at UP Health System - Portage. As a member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and creating a positive experience for our patients.
Key Responsibilities:
- Plan, implement, and coordinate rehabilitation programs for cardiac patients in collaboration with the cardiopulmonary rehabilitation team.
- Develop and maintain effective relationships with patients, families, and healthcare providers to ensure seamless care coordination.
- Provide education and counseling on healthy lifestyle habits, nutrition, and risk factor modification strategies to patients and their families.
- Collaborate with multidisciplinary teams to develop and implement individualized treatment plans that address the unique needs of each patient.
Requirements:
- Bachelor's Degree in Exercise Science or related field (or Associate's Degree in Nursing with relevant experience).
- Current certification as an American College of Sports Medicine (ACSM) Exercise Specialist (preferred).
- State of Michigan Registered Nurse licensure (for RNs) or certification as an Exercise Physiologist (for EPs).
- A minimum of 1 year of experience in cardiac rehabilitation (preferably in a hospital setting).
- Ability to work in a fast-paced environment and make sound decisions with minimal supervision.
- Excellent communication and interpersonal skills.
Salary : $50,000 - $80,000
