Job Overview:You'll coordinate hospice care for patients and families throughout Naperville and surrounding suburban communities west and south of Chicago. You'll independently manage your own caseload, develop patient-centered care plans, conduct assessments, and document clinical notes using NetSmart EMR (charting bedside with tablets). Regular collaboration with a dedicated interdisciplinary teamincluding physicians, social workers, and hospice aidesis part of your daily routine.
Key Responsibilities:Regularly assess patients, develop, and update individualized care plans.
Educate patients and families about hospice care and symptom management.
Coordinate patient care updates effectively with physicians and interdisciplinary team.
Accurately and promptly complete clinical documentation.
Provide compassionate end-of-life care and emotional support to patients and their families.
Actively participate in interdisciplinary meetings and care coordination.
Ensure compliance with Medicare, Medicaid, and HIPAA regulations.
Handle occasional on-call duties to maintain continuity of care.
Current Illinois RN license (unrestricted).
Minimum 12 years of RN experience required (hospice experience strongly preferred).
Comfortable working independently with minimal supervision.
Strong clinical judgment, patient-assessment skills, and compassionate communication.
Valid driver's license and reliable transportation.
EMR proficiency required (NetSmart EMR experience strongly preferred).
Competitive hourly rate: $41.32 $48.21 per hour (based on experience), plus a $10,000 sign-on bonus.
Flexible schedules available (including some evenings, weekends, and holidays).
Separate dedicated on-call positions are available to reduce on-call responsibilities for staff nurses.
Salary : $41 - $48
