About the Opportunity
A respected and fast-growing home health agency is hiring a full-time RN Case Manager to serve patients throughout Chesapeake and Suffolk, Virginia. You'll manage a dedicated caseload, conduct Start of Care visits, and work closely with therapists, aides, and providers to deliver coordinated, patient-centered care. This is a field-based role with mileage reimbursement and strong administrative support from a centralized office team.
Key Responsibilities

Conduct comprehensive nursing assessments and Start of Care (SOC) visits using OASIS

Create individualized care plans based on physician orders and patient goals

Provide skilled nursing interventions (e.g., wound care, medication management, patient/family education)

Coordinate interdisciplinary care with PT, OT, ST, MSW, and HHAs

Maintain timely, accurate documentation using EMR

Manage a caseload while maintaining compliance with CMS and agency standards
Requirements

Active Registered Nurse (RN) license in Virginia

Minimum of 1 year of clinical nursing experience (home health experience preferred)

Strong familiarity with OASIS documentation and SOC processes

Reliable transportation, valid driver's license, and auto insurance

Comfort with field-based EMR documentation (e.g., Homecare Homebase, Kinnser)
Schedule and Compensation

Full-time, Monday–Friday (8am–5pm) with some flexibility

Weekend on-call rotations may be discussed (compensated separately)

Salary range: $72,000 – $90,000, based on experience
