Responsibilities
- Lead and coordinate daily nursing activities to ensure high-quality care for our residents.
- Collaborate with the Registered Nurse to develop and implement personalized care plans.
- Monitor and document residents' health status, ensuring timely interventions when necessary.
- Provide compassionate support and guidance to residents and their families, fostering a warm community.
- Supervise and mentor nursing staff, promoting a culture of continuous learning and improvement.
- Ensure compliance with state and federal regulations, maintaining the highest standards of care.
- Act as a liaison between residents, families, and healthcare providers to facilitate effective communication and care coordination.
- Alternate on-call with facility leadership and nursing.
Requirements
- Must be licensed with the State of Wisconsin or Minnesota as an LPN, and must maintain current unencumbered licensure.
- Previous experience in a related field.
- Maintains current background check on file with the State of Minnesota or Wisconsin and agencies involved.
- Must have a valid Minnesota or Wisconsin Driver’s License.
- Drives company and personal vehicle in a safe manner and in accordance with MN and WI State Laws.
- Must be able to read, write, and communicate in English.
