Description

The Case Manager is responsible to assure adequate assessment and development of all program participants.

Essential Functions:

Responsible for:

  1. Helping people develop service plans in accordance with their wishes, dreams and desires
  2. Developing and facilitating the implementation of all programming related to an individuals' career goals.
  3. Providing crisis intervention as necessary.
  4. Facilitating the transition of appropriate referrals and ensuring adequate assessment of individuals.
  5. Communicating on a regular basis with representatives of funding agency.
  6. Conducting career exploration groups
  7. Conducting individual and family satisfaction surveys. Using data to improve the services for the individual
  8. Maintaining accurate and up to date case records.
  9. Supporting people to advocate for their needs within the organization.
  10. Helping people with job interviews and resume preparation
  11. Helping people understand their entitlements and how they relate to wages
  12. Providing follow-up services for individuals in competitive employment.
  13. Providing supervision for people prior to and after programming hours upon discharge and release to transportation company.
  14. Safety of the persons served under your supervision.
  15. Tracking IEE’s and Disability Determinations for the Employment/Ability One programs.
  16. Tracking monthly hours/goals for individuals served
  17. Provide job coaching as needed.

Qualifications


  1. Bachelor's Degree in Human Services or related field.
  2. Experience may be substituted in lieu of degree.

Mathematical Skills:

  1. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  2. Ability to compute rate, ratio, and percent, ability to create and interpret graphs.

Reasoning Ability:

  1. Ability to define problems, collect data, establish facts and draw valid conclusions.
  2. Ability to interpret an extensive variety of technical data in mathematical or diagram form and deal with several abstract and concrete variables.

Communication Demands:

  1. The ability to read, analyze and interpret journals, financial reports and legal documents.
  2. The ability to respond to common inquiries of complaints.
  3. The ability to effectively present information to the Board of Directors, employees, program participants and members of the business community.

Environmental Demands:

The work is performed in a non-smoking environment. The work is normally performed in a temperature controlled zone, however, there are exceptions. Preference will be given to candidates who can work the scheduled hours of 8am-4pm, M-F. This position pays $24.00 per hour and it is essential that the candidate can be prompt at 8 am and stay until 4 pm.
