JOB SUMMARY: The Case Manager for our Young adults and families program, you will play a major role in supporting single adults’ families with clinical conditions in scatter-site supportive housing. As a Case Manager in this program, you are responsible for ensuring that all single adults and families receive support tailored to their unique circumstances, helping them navigate challenges, achieve stability, and other resources. The Case Manager will be involved in a range of supportive activities, including but not limited to conducting detailed intakes, ongoing monitoring of progress, and ensuring the continuous maintenance of care plans.
EDUCATION AND WORK EXPERIENCE:
- MINIMUM: Bachelor’s Degree in Social Work, Human Services, or related field required
- MINIMUM: One (1) year of field experience providing housing and/or case management services to homeless populations, people with substance use disorders, and/or people with psychiatric illness.
- Ability to travel within Brooklyn by public transit.
- Up to 75% of traveling required.
DUTIES AND RESPONSIBILITIES:
Intake & Assessment
- Conducts intake assessments, quarterly re-assessments, and monthly (at minimum) home visits.
- Develops and implements initial, annual, and quarterly person-centered service plans for your caseload
- Conducts bi-annual case conferences with collateral providers.
- Provides referrals for various services as needed.
- Monitor clients' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes.
- Conduct regular follow-ups to assess progress and additional needs.
- Participate in internal and external case conferences.
Housing Support & Property Coordination
- Coordinates move-in for new clients and relocations for existing clients when necessary.
- Completes initial, annual, and updated apartment inspections.
Client Engagement
- Work with clients to break through barriers to service goals and to assist clients in advocating for themselves and in moving toward self-sufficiency.
- Monitor clients' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes.
Referral & Resource Linkage
- Refers clients to appropriate services including: Medical, dental, mental health, and substance use treatment Benefit programs such as SSI, SNAP, Legal assistance, education, employment, and training
- Follow up with clients and referral organizations regarding tenant contact and progress with services.
- Follow-up with clients and with referral organizations regarding tenant contact and progress with referral organization.
Community Engagement & Outreach
- Participate in community outreach efforts to engage clients and promote available services.
- Dedicate up to 65% or more of work hours to community engagement and fieldwork, including occasional home visits.
Crisis Intervention & Supportive Counseling
- Provides supportive counseling utilizing harm reduction and motivational interviewing approaches.
- Provides crisis intervention, de-escalation, and conflict resolution services.
Other Duties
- Other related duties as assigned based on program needs.
KNOWLEDGE SKILLS, AND ABILITES:
- Knowledge of issues related to substance use, recovery, mental health, and trauma-informed care.
- Familiarity with electronic medical records (EMR).
- Ability to work efficaciously and cooperatively with groups or individuals.
- Knowledge of fair housing regulations and laws pertaining to housing assistance programs.
- Ability to assess client needs and provide appropriate support and resources.
- Strong communication and interpersonal skills to effectively work with diverse populations.
- Excellent verbal and written communication skills, including the ability to convey complex information clearly and sensitively to diverse audiences.
- Comfortable with digital tools and software for project management, data tracking, and communication.
- Excellent organizational and record-keeping skills.
- Sound judgment, critical thinking, and problem-solving skills.
- Ability to multitask and manage multiple cases effectively.
- Experience in crisis management or addiction counseling is beneficial.
