JOB SUMMARY: Under the direct supervision of the Assistant Director, the Prevention Navigator/Testing Specialist is responsible for outreach, recruitment, and providing IDI Sister-to-Sister prevention education and STI/HCV testing services, with a focus on adult women identified as high-risk for STI/HCV/HIV, substance use, mental illness. Under the direct supervision of the Program Manager, The Prevention Navigator/Testing Specialist works along with the Health Educator and will conduct linkage and navigation services and PrEP support for qualifying clients.
This role has a variable schedule . General schedule 11 AM to 7PM preferred .
EDUCATION AND WORK EXPERIENCES:
- MINIMUM: High School Diploma or equivalent required
- Bachelor's Degree in public health, social work, or related field preferred.
- MINIMUM: Three (3) years of case management experience
- MINIMUM: Two (2) years of experience conducting HIV /Hep C testing and delivering phlebotomy services in an outreach setting.
- Ability to work flexible hours, including evenings and Saturdays.
- Bilingual in English and Spanish or another language spoken in Brooklyn communities is a plus.
- Valid NYS Driver’s License and ability to travel within Brooklyn by public transit.
Certification: Certified in Phlebotomy with a minimum of 1 year of experience in drawing blood in outreach settings.
Licensure: NYS Driver’s License in good standing and two years of driving experience strongly preferred
DUTIES AND RESPONSIBILITIES:
Outreach & Engagement
- Conduct field-based outreach in high-need areas (Central Brooklyn: Bedford Stuyvesant, Crown Heights, Brownsville, East New York, East Flatbush, Bushwick).
Testing & Screening
- Provide HIV/HEP C/STI urine testing, ensuring safety and infection control standards.
- Facilitate rapid HEP C testing and other point-of-care STI tests as appropriate.
- Provide HCV and STI screening for at least 150 clients annually.
Education & Counseling
- Deliver evidence-based education S2S, prevention counseling, and risk-reduction planning to 150 women aged 19 .
- Utilize trauma-informed practices in all interactions to ensure client safety, autonomy, and trust.
Navigation & Referrals
- Provide referrals and warm handoffs to appropriate medical and support services, including PrEP/PEP, HIV care, HEP C treatment, STI treatment, mental health, and substance use services.
- Provide navigation services to 95% of clients who test positive for HIV, STI, and HCV.
- Provide HIV referrals for 50 clients.
- Develop Action Plans based on risk assessments and assist with navigation services (accompaniment, follow-up, etc.) for newly diagnosed or out-of-care clients.
- Conduct SDOH screening to improve food and medical access.
- Link clients to services such as benefits, entitlements, food, transportation, and medical care.
Planning & Logistics
- Create a schedule to ensure supplies are available for all testing and outreach events.
Documentation & Reporting
- Maintain timely and accurate documentation in electronic health records and reporting systems (eCR and AIRS).
- Ensure 100% of all services are entered into the eCR and AIRS; documents are submitted after each service.
Teamwork & Professional Development
- Participate in regular team meetings, trainings, and continuous quality improvement activities.
- Attend community events, health fairs, and collaborate with local CBOs, shelters, and healthcare providers to increase program visibility.
- Conduct outreach and recruitment activities reaching 100 people per year.
- Create content for 12 social media posts.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Certified Phlebotomist, HIV/HCV Tester or Medical Assistant (Required)
- Previous experience delivering curriculum evidenced-based facilitation, a plus
- Providing clients with testing results and making referrals for treatment as needed
- Effective communication and documentation skills (written and electronic).
- Ability to work with people of various racial/ethnic backgrounds
- Computer literacy is required.
- Ability to withstand extended sitting, extended standing, stairs, and extended travel with or without reasonable accommodations.
- Must be willing to work weekends, nights, and long/flexible hours as required by the position
- Must be willing to conduct outreach/travel 75% of the time.
