**Job Title:** Home Health Aide II
The {company} is seeking a skilled and compassionate Home Health Aide II to join our team. This position plays a vital role in providing high-quality care to patients in their homes, promoting independence, dignity, and comfort.
Summary:
This position is responsible for providing necessary care to patients as designated by a written care plan developed by the RN Case Manager. The Home Health Aide II performs activities that assist patients in meeting basic human needs and activities of daily living under the direction of the RN.
Responsibilities:
- Assist patients with personal care and activities of daily living (ADLs), including bathing, oral hygiene, toileting, feeding, dressing, exercises, ambulation, transfers, and medications.
- Provide normal household services essential to patient care at home, such as meal preparation, laundry, and light housekeeping duties.
- Report any recognized changes in patient condition to the RN Case Manager or Registered Nurse (RN) designee.
- Communicate effectively with patients and families, adhering to confidentiality policies regarding patient scheduling, work assignments, agency-related work issues, and personal issues.
- Meet annual in-service requirements of 12 hours, with 6 hours focused on hospice-related topics.
- Participate in other agency activities, such as small group discussions, focus groups, committees, and preceptorships.
- Perform other duties as necessary to support Agency programs assigned by the Home Health Aide Supervisor.
Requirements:
- Certificate to practice as a Home Health Aide in the State of Connecticut or CNA with certification with the Connecticut State Nurse Aide Registry.
- Enrolled in or completed a Basic Approved Training Program as defined by Licensure Regulations or evidence of meeting exemption criteria.
- Previous experience as CNA/HCA preferred.
- Ability to read, write, and speak English.
- Ability to comprehend written and oral instructions in English.
- Current driver's license and access to a car.
- Possession of automobile insurance with minimum liability coverage of $20,000/$40,000.
- Flexibility, punctuality, and dependability.
- Excellent communication skills and ability to follow directions.
