$5,000 SIGN ON BONUS AVAILABLE FOR EXTERNAL CANDIDATES WITH THIS POSITION
This full time position will join the Heritage Youth and Family Support Services team to provide direct clinical services to Pathways to Success eligible youth. Pathways to Success is a program for Illinois Medicaid enrolled youth under the age of 21 who have complex behavioral health needs requiring intensive services and provides access to an evidence-informed model of intensive care coordination and additional home and community based services. Pathways to Success utilizes the evidenced-informed Care Coordination and Support model. This is a structured approach to care coordination that adheres to required procedures for child and family engagement, individual care planning, identifying and utilizing strengths and natural supports while monitoring progress and fidelity to the required process. Services will be provided in array of settings including individual’s home, community-settings, community agencies, schools, and Heritage.
Responsibilities Include:
- Develop and update as needed Crisis Prevention Plan and Crisis Safety Plan for each youth
- Complete and update as needed a Strengths, Needs, and Cultural Discovery for each youth
- Assemble a unique Child and Family team for each youth including formal and natural supports involved in the youth and family’s lives.
- Facilitate regular Child and Family team meetings for the purpose of identifying the needs and strengths of the youth and family, service planning, coordination of care, and regular review of the child and family’s progress toward meeting their goals.
- Utilize IM CANS to develop an individualized strengths-based service plan
- Contact youth as required to maintain program fidelity
Education and Experience:
Candidates are required to have minimal designation as a Mental Health Professional (MHP). A Mental Health Professional (MHP) is defined as 5 years supervised experience in behavioral health, Certified Recovery Support Specialist (CRSS), or a Bachelor’s Degree in psychology, social work, or human services.
