Job Responsibilities:
- Regular and predictable attendance is essential for this position.
- Train, schedule, and supervise assigned staff in carrying out individual treatment plans.
- Services may include sensory processing evaluation, ADL, IADL assessments, designing and monitoring subsequent treatment, identifying appropriate techniques and activities within the scope of OT practice, participating in staff education and training as needed.
- Promote community integration.
- Develop and implement groups and family education from established goals.
- Ensure assessment, goal setting, and treatment are person-centered.
- Collaborate with primary Case Holder to ensure goals are included in the Individual Plan of Service (IPOS) when necessary.
- Complete timely documentation of progress notes, initial evaluations, discharge summaries, home assessments, home programs, and consultation notes as indicated.
- Monitor individual progress and maintain accurate records of treatment through data collection and treatment reports as assigned.
- Serve as a member of an interdisciplinary team for assigned individuals. Participate in individual goal development, coordination of services, discharge planning, and family education as needed.
- Develop and maintain effective interpersonal relationships with all individuals.
- Attend and verbally present at team meetings, clinical reviews, staff meetings, and in-services. Attend departmental meetings and participate in program development.
- Order individual-specific treatment supplies and equipment.
- Follow established policies and procedures to ensure safety of staff and individuals.
- Provide supervision for Certified Occupational Therapy Assistants and/or OT students as required.
Job Education/Requirements:
- Minimum Education Required: Master's degree required.
- Licensed by State of Michigan MS, OTR/L.
- Two years of professional work experience in a clinical setting preferred.
- Demonstrated ability to communicate in verbal and written formats to meet position expectations.
- Valid driver's license required according to Hope Network policy.
Salary : $62,300 - $98,300
