Job Summary

Key Responsibilities:

• Train, schedule, and supervise assigned staff in carrying out individual treatment plans.
• Services provided may include sensory processing evaluation, ADL/IADL assessments, and designing and monitoring subsequent treatment.
• Promote community integration and develop groups and family education from established goals.
• Ensure assessment, goal setting, and treatment are person-centered and collaborate with primary case holders as needed.
• Complete timely documentation of progress notes, initial evaluations, and discharge summaries.
• Monitor individual progress and maintain accurate records of treatment through data collection and treatment reports.
• Serve as a member of an interdisciplinary team for assigned individuals and participate in individual goal development, coordination of services, and discharge planning.
