Job Responsibilities:
- Regular and predictable attendance is an essential requirement of this position.
- Train, schedule and supervise assigned staff in carrying out individual treatment plans.
- Services provided may include: Sensory Processing evaluation, performing the ADL, IADL and/or other appropriate clinical assessments, designing and monitoring subsequent treatment. Identification of appropriate treatment techniques, activities, and/or programs within the scope of OT practice. Participating in staff education and training as needed.
- Promote community integration.
- Develop and implement groups, and family education from established goals.
- Ensure assessment, goal setting and treatment (treatment may include individual and group setting) are person-centered.
- Collaborate with primary Case Holder to ensure goals are included the Individual Plan of Service (IPOS) when necessary.
- Complete timely documentation of progress notes, initial evaluations and discharge summaries, home assessments, home programs, and consultation notes as indicated.
- Monitor individual progress and maintain accurate records of treatment through data collection and treatment reports as assigned.
- Serve as a member of an interdisciplinary team for assigned individuals. Participate in individual goal development, coordination of services, discharge planning and family education as needed.
- Develop and maintain effective interpersonal relationships with all individuals.
- Attend and verbally present at team meetings, clinical reviews, staff meetings and in-services. Attends departmental meetings and participates in program development.
- Order individual specific treatment supplies and equipment.
- Follow established policies and procedures to ensure safety of staff and individuals.
- Provide supervision for Certified Occupational Therapy Assistants, and/or OT students as required.
Job Education/Requirements:
- Minimum Education required: Master's degree required.
- Licensed by State of Michigan MS, OTR/L.
- Two years of professional work experience in a clinical setting preferred.
- Demonstrated ability to communicate in verbal and written formats to meet position expectation.
- Valid driver's license required according to Hope Network policy.
