What are the responsibilities and job description for the Licensed Practical Nurse position at Interim HealthCare of the Twin Cities?
As a Home Care Licensed Practical Nurse (LPN) at Interim HealthCare of the Twin Cities, you will have the opportunity to work independently and autonomously in patients' home environments. Your primary responsibilities will include providing care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.
You will be working full-time days, with a primary schedule of Monday - Friday from 8:00am - 5:00pm. Weekend and holiday rotation is dependent upon staffing levels, with approximately two holidays per year and one weekend every 4 - 6 weeks.

The territory for this position will involve working with patients in their homes in the Southwest Metro area, south of Highway 394 and west of Interstate 35W.
To succeed in this role, you will need:
- A current LPN license in good standing in the State of Minnesota
- At least 2 years of experience as an LPN
- Able to travel within the geographic area serviced by the office, with a valid driver's license, reliable transportation, and proof of auto insurance
- CPR certification
As an LPN at Interim HealthCare, you will have the opportunity to:
- Participate in ongoing interdisciplinary assessment of the patient
- Provide services ordered by the physician as indicated in the plan of care
- Recognize and report changes in the patient's condition to the RN
- Counsel and educate the patient and family in meeting healthcare-related needs
- Complete timely and accurate clinical notes, including addressing patient progress
