What are the responsibilities and job description for the Licensed Practical Nurse position at Interim HealthCare of the Twin Cities?
As a Home Care Licensed Practical Nurse (LPN), you will enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment.
You will provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.
Key Responsibilities:
- Participate in ongoing interdisciplinary assessment of the patient
- Provides services that are ordered by the physician as indicated in the plan of care
- Recognizes and reports changes in the patient's condition to the RN
- Counsels and educates the patient and family in meeting healthcare related needs
- Completes timely and accurate clinical notes including addressing patient's progress
This position involves working in the field helping patients in their home setting. The general territory for this position would be in the Southwest Metro area, serving patients south of highway 394 and west of 35W.
Requirements:
- Current license as an Licensed Practical Nurse (LPN) in the State of MN, with an active and good standing license
- 2 years experience as an LPN
- Able to travel within geographic area serviced by the office, with valid driver's license, reliable transportation, and proof of auto insurance
- CPR Certification
Salary : $60,000 - $76,800
