What are the responsibilities and job description for the Licensed Practical Nurse position at Interim HealthCare of the Twin Cities?
As a Home Care Licensed Practical Nurse (LPN), you will have the opportunity to provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety in the comfort of patients' own homes.
The ideal candidate will have a current license as an LPN in the State of MN, 2 years of experience as an LPN, and be able to travel within the geographic area serviced by the office. They should also possess CPR Certification and have valid driver's license, reliable transportation, and proof of auto insurance.
Responsibilities:
- Participate in ongoing interdisciplinary assessment of the patient
- Provide services that are ordered by the physician as indicated in the plan of care
- Recognize and report changes in the patient's condition to the RN
- Counsel and educate the patient and family in meeting healthcare related needs
- Complete timely and accurate clinical notes including addressing patient's progress
