What are the responsibilities and job description for the Licensed Practical Nurse position at Interim HealthCare of the Twin Cities?
As a Home Care Licensed Practical Nurse (LPN), you will have the opportunity to work independently and autonomously in a patient-centered environment. Your role will involve providing care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.
Key Responsibilities:
- Participate in ongoing interdisciplinary assessment of the patient
- Provide services that are ordered by the physician as indicated in the plan of care
- Recognize and report changes in the patient's condition to the RN
- Counsel and educate the patient and family in meeting healthcare related needs
- Complete timely and accurate clinical notes including addressing patient's progress
Schedule:
The primary hours for this position would be Monday - Friday from 8:00am - 5:00pm, with weekend and holiday rotation dependent upon staffing levels. You can expect to work approximately two holidays per year and one weekend every 4-6 weeks.

Territory:
This position will work in the field helping patients in their home setting. The general territory for this position is the Southeast Metro, which includes the cities south of highway 94 and east of 35W.
Requirements:
- Current license as an Licensed Practical Nurse (LPN) in the State of MN
- 2 years experience as an LPN
- Able to travel within geographic area serviced by the office
- Valid driver's license, reliable transportation, and proof of auto insurance required
- CPR Certification
Salary : $47,352 - $76,195
