Transition Specialist
Part-Time: 20 Hours
Starting Pay Rate: $18.27 per hour

Job Duties:
The successful candidate will be expected to:
- Meet with individuals who desire to transition form an institution to living in the community; identify what services are needed and assist with those services.
- Develop a person-centered transition plan.
- Identify and remove barriers for individuals transitioning to living in the community.
Education:
- A High School Diploma or equivalent required.
Knowledge, Skills & Abilities:
- Experience working with individuals with disabilities, either professionally or personally.
- Strong advocacy skills.
- Knowledge of nursing home transition process and the Americans with Disabilities Act.
COVID-19 vaccination required for this position. RCIL will engage in the accommodation process when an exemption is requested.
