 **$5,000 Sign-On Bonus**
Purpose: Under the direction of the Director, Nursing, the RN functions as a member of the interdisciplinary health care team in providing the highest quality of care to the patient.

Detail of Key Job Responsibility Areas 
 

1. Coordinate Care for Trillium Clients

  1. Work with the interdisciplinary health care team to facilitate care 
  2. Work with interdisciplinary health care team to keep patients and parents engaged
  3. Actively participate in quality improvement initiatives
  4. Demonstrate positive communication and teamwork within clinic and between departments

 

2. Clinical Responsibilities                  

  1. Assist providers in delivering patient care.
    1. Explain procedures and treatments to patients to gain cooperation, understanding and allay apprehension
    2. Aid providers during examinations and treatments, as requested
    3. Observe patient and record significant reactions/conditions. Notify providers of reaction to drugs, treatments.
  2. Confer with providers; assist with written prescriptions, refills, phone-ins to pharmacies, and documentation on medication list
  3. Triage daily patient related telephone calls. Assign priority rating and refer to providers as needed. When appropriate resolve patient problem by communicating care instructions and/or advice.  
  4. Administer treatments, injections, immunizations 
  5. Patient education on self-injection, new medications, or other treatments or issues related to care
  6. Vision / Hearing screening 
  7. Front Desk Billing
  8. Autoclave 
  9. Rapid strep, rapid flu, rapid mono, rapid hcq, Lead, Hemoglobin, PPD application and readings.
  1. As needed, perform duties of Medical Office Assistant taking vitals and updating patient information, collecting samples from patients such as urine. Perform tests such as EKG as required
  2. Maintain supplies and vaccines, including the stocking and ordering of patient specific supplies as needed.  Dispose of expired or damaged supplies
  3. Perform routine scheduled inventory of patient medications, sample drugs and injectable medications, O2, and testing of defibrillators
  4. Flex as needed to cover nursing absences
  1. Complete a minimum of 10 hours of continuing education annually
  2. Practice within the NYS RN scope of practice and maintain annual competencies

 

  3.   Documentation

  1. Document all patient care clearly, thoroughly, accurately, and timely
  2. Order and maintain inventory of medications/ supplies    
  3. Maintain ongoing list of patients’ preventative care needs             

 

  4.  Administrative

  1. Administer employee health information. Administer immunizations and PPD tests as needed. Record results in employee health file. Establish and maintain schedule to ensure all employee information and tests/injections are updated and administered on a timely basis as required by Health Department regulations.
  2. Participate in the quality assurance program monthly CQI/QA activities as requested
  3. Participate in clinical team and case management activities          
  4. Participate in the training and clinic orientation of new staff

 

5. Miscellaneous

Qualifications

 
Must possess valid NYS license. Previous experience working in a hospital or clinic setting required. Knowledge of HIV/AIDS extremely helpful. Excellent written/oral communication and computer skills are required.  


Physical Requirements


While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
