Purpose:
Provides program oversight for testing, outreach, education and linkage to care as well as direct supervision of staff. Is the co-coordinator for all testing services at all Trillium Health locations. Manages the outreach schedule to focus on high-volume testing/education/linkage to care areas and focuses on priority populations.

Community Outreach

  • Responsible for oversight of all offsite outreach opportunities, inclusive of education and testing events, to ensure they are fully staffed, supplies are available as necessary and those served are within targeted populations. Coordinate the development and maintenance of a process for data collection and analysis for outreach events that includes measures of success.
  • With the guidance of the Sr. Manager, Community Prevention Services, address gaps in outreach and testing by identifying external partners or places to provide services.
  • Work collaboratively with the Clinical Informatics and Business Intelligence teams to ensure there are efficient ways to collect and report out data on testing services and linkages to care utilizing the EMR.
  • Collaborate with program leadership to incorporate evidence-based interventions, as appropriate, during encounters.
  • Be well versed in all programs in which testing and outreach can be a facilitator for linkage to care i.e. PrEP/PEP, HIV Rapid Start, HCV treatment, etc.
  • Demonstrate developing knowledge of HIV/AIDS and other health-related issues.


Testing Coordination

  • Along with the Sr. Manager, Community Prevention Services, provide coordinated oversight of Trillium Health CLIA-waived, community based testing practices (including HIV/STI/HCV) administered within the community or onsite at Trillium Health locations.
  • In coordination with the Sr. Manager, Community Prevention Services, and department leadership, develop and revise policies and procedures per funding, state and organizational guidelines.
  • Provide coordinated training to all staff on various testing products and procedures with the Manager, Community Prevention Services.
  • In coordination with the Manager, Community Prevention Services, liaison between Trillium Health departments regarding testing policies and procedures and linkages to care.
  • In coordination with the Sr. Manager, Community Prevention Services, provide quality assurance for all HIV/STI/HCV related testing and documentation including ongoing competencies of all testers.
  • In coordination with the Sr. Manager, Community Prevention Services, oversee ordering and inventory of testing supplies.
  • Utilize the EMR to ensure adequate and efficient documentation of testing services and linkages to care.
  • In coordination with the Sr. Manager, Community Prevention Services, support electronic medical record training to all testing staff.
  • In coordination with the Sr. Manager, Community Prevention Services, maintain testing competencies for self and provide direct services as needed.


Supervision

  • Provide direct supervision to staff including one-on-one supervision.
  • Responsible for completing 6-month evaluations and annual performance appraisals.
  • Provide ongoing feedback to staff on strengths, areas for improvement and areas of concern.
  • Recommend and provide opportunities for training and education to staff to strengthen their skills in their current roles, areas that need improvement or in support of the organization, program and staff’s goals.
  • Responsible for organizing and facilitating new hire onboarding and training.
  • Manage timecards and PTO requests for direct reports.
  • Schedule and facilitate testing team meetings as needed.


Administrative

  • In conjunction with Program Support and Data Entry staff, enter, compile and report program data as required by program and/or funder.
  • Function as liaison with internal staff and other community agencies to promote testing and linkage to care
  • Maintain inventory for testing supplies and incentives as needed.
  • Participate in training and continuing education initiatives to build understanding of evolving treatment and service delivery issues.
  • Work with department leadership in developing ongoing sexual health education curriculum
  • Able to provide quality assurance for all HIV/STI/HCV related testing and documentation

  • Attend required training as directed by New York State Department of Health and direct supervisor.
  • Attend scheduled meetings with regional New York State Department of Health representatives and other project partners
  • Other specific duties as assigned.


Qualifications

  • Knowledge of HIV and other sexually transmitted infections required
  • At least two years of experience supervising or leading a team preferred
  • At least three years’ experience in Health Education or related field required
  • Bachelor’s degree in nursing, Health and Human Services or related field preferred or equivalent experience commensurate with lower level of education. Minimum High School diploma or GED required
  • Bilingual in Spanish and/or ASL desired


Physical Requirements

While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
