Key Responsibilities:- Conduct comprehensive assessments of clients’ situations to develop individualized service plans.
- Provide counseling and emotional support to clients facing life transitions, health issues, and other personal challenges.
- Collaborate with healthcare providers, community organizations, and family members to coordinate services and resources.
- Advocate for clients’ rights and access to necessary services, ensuring they receive the best possible care.
- Document client interactions, progress, and outcomes in compliance with legal and ethical standards.
- Active LCSW (Licensed Clinical Social Worker) or LMSW (Licensed Master Social Worker) in the state.
- Experience working in clinical settings, with a focus on community outreach and resource connections.
- Excellent communication and interpersonal skills with a compassionate and nonjudgmental approach.
- Ability to work independently and as part of a multidisciplinary team.
- Strong organizational skills and proficiency in using electronic case management systems.
