Job Description:
As a Licensed Practical Nurse (LPN) with Interim HealthCare of the Twin Cities, you will have the opportunity to provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety to patients in their home environment.
You will participate in ongoing interdisciplinary assessment of the patient, provide services ordered by the physician as indicated in the plan of care, recognize and report changes in the patient's condition to the RN, and counsel and educate the patient and family in meeting healthcare related needs.
Our office is located in Roseville, MN, and the general territory for this position is in the Southwest Metro, south of highway 394 and west of 35W.
Responsibilities:
- Provide care coordination, disease and medication management
- Skilled wound care, catheter management, ostomy education
