As a Licensed Practical Nurse (LPN) with Interim HealthCare of the Twin Cities, you will have the opportunity to provide high-quality care to patients in their home environment.
Job Responsibilities:
- Provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety
- Participate in ongoing interdisciplinary assessment of the patient
- Provides services that are ordered by the physician as indicated in the plan of care
- Recognizes and reports changes in the patient's condition to the RN
- Counsels and educates the patient and family in meeting healthcare related needs
Requirements:
- Current license as an LPN in the State of MN
- 2 years experience as an LPN
- Able to travel within geographic area serviced by the office
- CPR Certification
