Job Summary:
As a Licensed Practical Nurse (LPN) with Interim HealthCare of the Twin Cities, you will have the opportunity to work independently and autonomously in home care settings, providing one-on-one care to patients in their homes. Your primary responsibilities will include providing care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.
- Provide care coordination and services as ordered by physicians
- Recognize and report changes in patient condition to RNs
- Counsel and educate patients and families on healthcare-related needs
- Participate in ongoing interdisciplinary assessment of patients
Work Environment:
This position will work in the field, visiting patients in their home settings. Our office is located in Roseville, MN, and the general territory for this position includes the Southwest Metro area, south of Highway 394 and west of 35W.
Requirements:
- Current license as an LPN in the State of MN
- 2 years experience as an LPN
- Able to travel within geographic area serviced by the office
- CPR certification
